<p>Having stepped up lifecycle management for its leading breast cancer monoclonal antibody, Herceptin, Roche hopes to encourage uptake of a more convenient formulation of the drug. This strategy could help to conserve its share of an increasingly competitive market, but may be challenging to implement due to concerns over compliance and financial incentives to prescribe intravenous cancer drugs. <p>Roche has invested roughly CHF190m ($183m) at two production sites to manufacture devices that will allow patients to self-administer a subcutaneous formulation of Herceptin (trastuzumab; Roche/Genentech/Chugai). <span id="more-3410"></span></p>
<p>One site will supply the devices for a Phase III study which could support approval of the subcutaneous formulation. In this study, 552 patients with stage I-IIIc breast cancer will receive chemotherapy in combination with either subcutaneous or intravenous Herceptin before surgery, followed by 10 three-weekly infusions of subcutaneous or intravenous Herceptin.</p>
<p>Herceptin is a monoclonal antibody directed towards the human epidermal growth factor receptor-2 (HER-2), which is over-expressed in approximately 25% of the breast cancer population. In its intravenous formulation, Herceptin has established itself as a leading treatment for HER-2-positive breast cancer, both for patients with early-stage (stage I-III) and metastatic (stage IV) forms of the disease. In 2008, global sales of the drug totaled $4.7 billion.</p>
<p>Herceptin&#8217;s current intravenous formulation requires patients to receive the drug in hospital via a one-hour infusion. The subcutaneous formulation (developed in partnership with Halozyme Therapeutics) would increase convenience, allowing patients to self-administer the drug at home via a five-minute infusion. This is important given that Herceptin is administered for up to a year in early-stage breast cancer. Additionally, this would help to reduce costs associated with patient hospitalization and could improve the drug&#8217;s side-effect profile by reducing infusion reactions.</p>
<p>Roche will hope that these factors increase brand loyalty and protect Herceptin&#8217;s market share against potentially fierce competition from GlaxoSmithKline&#8217;s oral HER-2 inhibitor, Tykerb (lapatinib), which is approved for metastatic breast cancer and is currently in Phase III trials for early-stage HER-2-positive breast cancer. Furthermore, switching patients to a subcutaneous formulation could help insulate against potential competition from biosimilar versions of intravenous Herceptin. As the lucrative monoclonal antibody market approaches maturity, such lifecycle management activities are likely to become increasingly common.</p>
<p>However, Roche could find it challenging to encourage uptake of subcutaneous Herceptin because it will make it more difficult for physicians to ensure patient compliance. Indeed, patient compliance can be a problem for some orally administered anticancer drugs. In the US, financial incentives for oncologists to prescribe intravenous drugs could further restrict uptake of the subcutaneous formulation. The ongoing Phase III study may therefore have to show that the more convenient subcutaneous formulation also has superior efficacy in order to drive appreciable uptake.</p>

<p>The US House of Representatives has passed legislation paving the way for major healthcare system reform designed to substantially decrease the number of individuals without health insurance. Despite its potential benefits, it believes that the reform is a mixed blessing for the pharmaceutical industry as cost-containment pressures are set to intensify in the long term. <p>The landmark bill will overhaul the US healthcare system, aiming to reduce the number of individuals without healthcare insurance, improve access to healthcare and curb escalating costs. The bill focuses on health insurance system reform, specifically: expanding Medicaid insurance coverage for the poor; implementing stricter rules for private health insurers; outlining the creation of an individual mandate to purchase insurance; and introducing health insurance premium subsidies. <span id="more-3401"></span>As a result of the legislation, it is expected that 32 million more Americans will receive healthcare insurance coverage over the next 10 years. However, whether the reform will manage to contain escalating healthcare costs is questionable.</p>
<p>With the introduction of a ban on refusal of insurance coverage for pre-existing conditions and removal of annual or lifetime coverage limits, the health insurance industry will be hit hardest by the reform. Its profitability is in jeopardy, despite the increasing number of customers. However, the exact impact on insurers will depend on how many healthy individuals purchase insurance coverage, thus reducing the over-representation of less healthy individuals in the risk pool. In addition, without a government-run public option competing with private insurers, and without controls on premiums, insurers may pass on the costs to their current customers, resulting in an increased cost burden for middle-income Americans. This may well prove to be an unpopular strategy given the current spotlight on extraordinary premium hikes, and could prompt the introduction of further restrictions on insurers.</p>
<p>For the pharmaceutical industry, the passing of the biggest US healthcare reform in 40 years is a double-edged sword. The industry will benefit from increased patient access to healthcare as a result of the reform and, moreover, will be grateful for some certainty in the future market outlook. Indeed, despite a gloomy outlook at the beginning of Obama&#8217;s presidency, pharmaceutical firms will ultimately benefit from the reform thanks to early engagement in the negotiation process.</p>
<p>However, the reform represents a significant change in current practices, and it believes that there remain a number of potential short- and long-term dangers for pharmaceutical companies.</p>
<p>In order to pay for the reform, the industry will have to contribute higher fees and increased drug price discounts and rebates. In the short term, it is likely that the imposed 50% discount for Medicare Part D beneficiaries in the coverage gap (the &#8216;donut hole&#8217;) and increased Medicaid drug rebates, in addition to raised industry fees, will lead to a market dip. This could not come at a worse time for an industry facing patent expiries in 2011 that are set to wipe tens of billions of dollars off annual revenues.</p>
<p>These negative effects will be offset from 2015 as revenues begin to rise, driven mainly by the increase in the number of insured individuals and the resulting increase in drug consumption. An additional boost to sales volume will be derived from the expected increased drug consumption of Medicare Part D beneficiaries in the donut hole as the coverage gap is closed gradually from 2010 to 2020. With lower out-of-pocket costs, beneficiaries will be less likely to forego filling their prescription in addition to having less of an incentive to switch to generics once they reach the donut hole. Furthermore, the industry will benefit from the longer market exclusivity given to branded biologics, which are set to drive its future growth.</p>
<p>While this will be beneficial in the medium term, greater government participation in provision of healthcare will inevitably ensue. This will be unwelcome in both the pharmaceutical and health insurance industries. In addition, growing cost strains on both public and private payers will lead to increased negative pressures on the pharmaceutical industry in the US, decreasing the overall value growth of the market. It cannot be ruled out that unfavorable measures such as drug re-importation and Medicare Part D price negotiations may be introduced as healthcare costs start to bite, or may more likely be used to leverage greater compromises from the industry.</p>
<p>Generics providers will also question the benefits of the reform, despite the promise of higher sales volume from an increase in the number of insured individuals. Specifically, the 12 years of market exclusivity given to branded biologics (which is substantially longer than the usual five years given to small molecule drugs) will be a concern given the focus on biosimilars as major revenue drivers in the future.</p>
<p>The reform bill still faces one final hurdle. The Senate has to pass the reconciliation bill, containing amendments to the Senate bill made by the House of Representatives, and it is possible that Republicans may stall this process.</p>
<p>Should the bill be signed into law, it will become a piece of landmark healthcare legislation with massive implications for pharmaceutical companies and health insurers alike. However, the healthcare reform process still has a long way to go, and government agencies and state legislators are yet to determine many of the finer details.</p>

			<content:encoded><![CDATA[<p>The humble little cannabis sativa plant is the center of one of the hottest issues between politics, religion, and medicine in all of history. Amidst all of the controversy and regardless of where they stand on the issue, medical professionals must find themselves curious about it. Is marijuana the miracle drug it is purported to be in some circles, or is this a lot of media hype? And likewise, is marijuana the dangerous, addicting road to drug-addled debauchery that the moral crusaders would have us believe, or is that, too, a lot of hype?</p>
<p>The claims about the medical benefits of marijuana, if all true, would instantly make it one of the most versatile drugs in history. So far, researches have proposed that it could treat Alzheimer&#8217;s disease, Amyotrophic Lateral Sclerosis, diabetes mellitus, dystonia, fibromyalgia, gastrointestinal disorders, gliomas, hepatitis C, hypertension, incontinence, osteoporosis, pruritis, rheumatoid arthritis, sleep apnea, and Tourette&#8217;s syndrome. The reports of new finding pour in every day. The latest claim is that cannabis extracts may shrink brain tumors and other cranial cancers by blocking the growth of the blood vessels which feed them.</p>

<p>Beyond medical treatment of existing ailments, marijuana is also claimed to be preventative of a whole larger spectrum of problems as well. It is supposed to prevent blindness, migraines, and depression. It is said that marijuana has both stalled the growth of and eliminated brain tumors in rats. Oh, and it fights hardened arteries and actually helps cleanse the lungs, preventing tumors. Come on, can it really be all that? What else, does it make whites whiter and colors brighter?</p>
<p>It could be argued that much of the news of marijuana&#8217;s health treatment uses are motivated by the desire to legalize it for recreational usage as well. After all, we are talking about a cheap, easily cultivated, low-dosage psychoactive narcotic with a mellow high, almost no side effects, and a low physical addiction rate.</p>
<p>Its effects can be felt merely by eating it, although smoking and, increasingly, vaporizing it are the preferred methods of recreational consumption. There have so far been no reported deaths or reports of permanent injuries sustained as a result of a marijuana overdose. There&#8217;s no sense fooling ourselves &#8211; people want to party, and those with a recreational motive might make up any fact they could get their hands on to justify their right to party.</p>
<p>A marijuana dosage and its associated high produce a group of psychoactive effects. The inebriated state is somewhat comparable to the effects of alcohol. Temporary impairment of memory functioning, motor skills, and cognition are all demonstrated. However, marijuana&#8217;s effects are largely dependent on the mood of the individual at the time of dosage. In short, a body at rest tends to stay at rest, and a body in motion tends to stay in motion. This variable effect is known as &#8220;set and setting&#8221;.</p>
<p>The illegality of usage and its demonized reputation as a gateway drug have created the flack in the opposite direction. Regarding international laws, it is legal for all uses in just under 10% of the world, decriminalized for medical use in about 50% of the world, illegal but unenforced in 10% of the world, and fully illegal in 30% of the world. Part of that is misleading, however &#8211; it is either decriminalized or legal for medical purposes in 18 states of the US as far as state laws go, but still illegal on a Federal level. Also, in parts of Europe, it is legal only in designated areas, so the whole country doesn&#8217;t really count.</p>
<p>Finally, &#8220;decriminalized&#8221; isn&#8217;t the same thing as &#8220;legal&#8221;, but only means that possession is punishable by a citation, confiscation, and a fine instead of by prison time.</p>
<p>It is probably logical to conclude that there is a substantial amount of propaganda and misinformation from both marijuana advocates and opponents due to the legal issues of marijuana, including the legal and political constraints on marijuana research, the firmly held beliefs of the public, and the religious beliefs against it. The most confounding factor in marijuana research is said to be the prevalent usage of other recreational drugs, including alcohol and tobacco, in the research subjects.</p>
<p>Such research complications demonstrate the need for studies on marijuana where stronger controls are used, and investigations into the symptoms of marijuana use that may also be caused by tobacco or other drugs. Much marijuana research in the United States is funded by government agencies that publish position papers which only cite research studies that report negative consequences of marijuana use. In light of this, some people question whether these agencies are making an honest effort to present an accurate, unbiased summary of the evidence, or whether they are slanting the results in favor of keeping it criminalized.</p>
<p>The medical field is caught in the middle of an ethical and scientific tug of war, which should be a familiar feeling to many. After the huge controversies over abortion, stem cell research, scientific findings pointing to evolution, cloning and gene therapy, cosmetic surgery, and artificial insemination, one could hardly blame medical researchers if they got disgusted and quit &#8211; or at least packed their bags and left the country for a place where they could do their job without quite so much politics getting in the way.</p>
<p>The medical marijuana controversy has reached a particularly poignant milestone just recently. The case is one Ms. Angel Raich, who is a long-term and terminally-ill medical cannabis patient in California. Ms. Raich is an Oakland mother of two who suffers from scoliosis, a brain tumor, chronic nausea and other ailments and complications.</p>
<p>On her doctor&#8217;s advice, she eats or smokes marijuana every couple of hours through the day to ease her pain and bolster her appetite as conventional drugs did not work. In spite of this, the hospice where she was staying got raided, and she faces criminal prosecution. Her doctor has protested that marijuana is the only thing keeping her alive, and the federal appeals court has now handed down the decision that she is to be prosecuted by the law &#8211; anyway!</p>
<p>Not only that, but this is not even the first time that medical marijuana has been denied as a treatment option, even to those terminally ill and even in states where it is legal for medical usage. This sobering state of events highlights the bind that the medical profession is in. There has seldom in history been such a case where criminal law and the Hippocratic Oath have been at such odds with each other.</p>

<p>Phase II clinical trials of tanezumab in osteoarthritis, chronic low back pain and interstitial cystitis have demonstrated the drug&#8217;s analgesic efficacy across these three chronic pain settings. <p>Pfizer has announced positive results from three Phase II trials in pain at the American Academy of Pain Medicine&#8217;s 26th Annual Meeting. The randomized, double-blind, placebo-controlled six-week trials in patients with chronic low back pain (CLBP) and interstitial cystitis receiving a single intravenous infusion of tanezumab (200mcg/kg) met their primary endpoints of significantly superior pain relief over placebo.<span id="more-3355"></span> The CLBP study also included a third treatment group receiving naproxen twice daily, and tanezumab showed a greater analgesic effect than this competitor. In the 16-week osteoarthritis trial, patients were randomized to received tanezumab (10mcg/kg, 25mcg/kg, 50mcg/kg, 100mcg/kg or 200mcg/kg) or placebo. All doses of the drug achieved a reduction in pain while at the same time demonstrating a favorable safety profile.</p>
<p>It estimates that 140 million people suffer from chronic pain across the seven major markets (the US, Japan, France, Germany, Italy, Spain and the UK). Chronic pain treatments include narcotic analgesics and non-steroidal anti-inflammatory drugs (NSAIDs); however, these therapies suffer from high abuse potential and limited efficacy. Given that pain will become more prevalent in the future due to the aging population, the chronic pain market continues to represent an attractive commercial opportunity for pharmaceutical developers.</p>
<p>Tanezumab is a humanized monoclonal antibody (mAb) against nerve growth factor (NGF) and is in position to become Pfizer&#8217;s first pain medication since its neuropathic pain treatment Lyrica (pregabalin) was approved in 2005. With strong efficacy demonstrated in different pain settings, the drug is expected to be the first major novel therapy in the management of pain for a number of years. However, while tanezumab offers good evidence as a &#8216;proof of concept&#8217; that interfering with the NGF pathway provides an analgesic effect, strong competition exists in the pipeline: Johnson &amp; Johnson&#8217;s JNJ-42160443; Sanofi-Aventis&#8217; and Regeneron&#8217;s Phase II drug REGN475; and Abbott&#8217;s PG110, which entered Phase I clinical trials in mid-2009.</p>
<p>Although the NGF class uses a novel mechanism for the treatment of pain, it believes that drug safety profiles may limit the patient population. Adverse events documented thus far include an abnormal peripheral sensation (tingling and numbness) and muscle aches. Moreover, given that NGF is involved in the development of the nervous system, reproductive toxicology issues will prevent its use in women of childbearing age. Another limiting factor could be cost: It expects that these therapies will be priced similarly to the biologics available to treat rheumatoid arthritis, and as such will be far more expensive than existing pain treatments. This will lead to insurance companies effectively forcing clinicians to try current therapies first, with NGF drugs held back as last-line alternatives.</p>
<p>With the investment made by major pharmaceutical companies, the success of tanezumab (and that of the whole NGF class) will be dependent on the characterized risk-benefit profile. Phase II data suggest efficacy in a number of pain conditions. However, at present no patient has received at least a year&#8217;s treatment with tanezumab, so its long-term safety profile remains unknown. its recommends that NGF drug developers also pursue indications in acute pain, in case the chronic use of these therapies proves intolerable. With the pain market totaling $25.4 billion in 2008 across the seven major markets and the lack of efficacious pain treatments, NGF drugs could capture a considerable proportion of this market. Following analysis of Phase III trials that are due to finish in 2011, it anticipates the launch of tanezumab in 2012 as the first biologic for the management of pain.</p>
