Pfizer’s novel NGF pain drug is potential game changer

Pfizer has announced positive results from three Phase II trials in pain at the American Academy of Pain Medicine’s 26th Annual Meeting. The randomized, double-blind, placebo-controlled six-week trials in patients with chronic low back pain (CLBP) and interstitial cystitis receiving a single intravenous infusion of tanezumab (200mcg/kg) met their primary endpoints of significantly superior pain relief over placebo. The CLBP study also included a third treatment group receiving naproxen twice daily, and tanezumab showed a greater analgesic effect than this competitor. In the 16-week osteoarthritis trial, patients were randomized to received tanezumab (10mcg/kg, 25mcg/kg, 50mcg/kg, 100mcg/kg or 200mcg/kg) or placebo. All doses of the drug achieved a reduction in pain while at the same time demonstrating a favorable safety profile.

It estimates that 140 million people suffer from chronic pain across the seven major markets (the US, Japan, France, Germany, Italy, Spain and the UK). Chronic pain treatments include narcotic analgesics and non-steroidal anti-inflammatory drugs (NSAIDs); however, these therapies suffer from high abuse potential and limited efficacy. Given that pain will become more prevalent in the future due to the aging population, the chronic pain market continues to represent an attractive commercial opportunity for pharmaceutical developers.

Tanezumab is a humanized monoclonal antibody (mAb) against nerve growth factor (NGF) and is in position to become Pfizer’s first pain medication since its neuropathic pain treatment Lyrica (pregabalin) was approved in 2005. With strong efficacy demonstrated in different pain settings, the drug is expected to be the first major novel therapy in the management of pain for a number of years. However, while tanezumab offers good evidence as a ‘proof of concept’ that interfering with the NGF pathway provides an analgesic effect, strong competition exists in the pipeline: Johnson & Johnson’s JNJ-42160443; Sanofi-Aventis’ and Regeneron’s Phase II drug REGN475; and Abbott’s PG110, which entered Phase I clinical trials in mid-2009.

Although the NGF class uses a novel mechanism for the treatment of pain, it believes that drug safety profiles may limit the patient population. Adverse events documented thus far include an abnormal peripheral sensation (tingling and numbness) and muscle aches. Moreover, given that NGF is involved in the development of the nervous system, reproductive toxicology issues will prevent its use in women of childbearing age. Another limiting factor could be cost: It expects that these therapies will be priced similarly to the biologics available to treat rheumatoid arthritis, and as such will be far more expensive than existing pain treatments. This will lead to insurance companies effectively forcing clinicians to try current therapies first, with NGF drugs held back as last-line alternatives.

With the investment made by major pharmaceutical companies, the success of tanezumab (and that of the whole NGF class) will be dependent on the characterized risk-benefit profile. Phase II data suggest efficacy in a number of pain conditions. However, at present no patient has received at least a year’s treatment with tanezumab, so its long-term safety profile remains unknown. its recommends that NGF drug developers also pursue indications in acute pain, in case the chronic use of these therapies proves intolerable. With the pain market totaling $25.4 billion in 2008 across the seven major markets and the lack of efficacious pain treatments, NGF drugs could capture a considerable proportion of this market. Following analysis of Phase III trials that are due to finish in 2011, it anticipates the launch of tanezumab in 2012 as the first biologic for the management of pain.

EASD 2009: Lantus defended amid allegations of increased cancer risk

Although the European Association for the Study of Diabetes (EASD) recommended the continued use of Lantus in type 1 and type 2 diabetics, it has called for increased research into the relationship between insulin therapy – particularly Lantus (insulin glargine; Sanofi-Aventis) – and the risk of cancer.

It was postulated during the discussion at the 2009 EASD meeting that insulin use may be related to the increased incidence of cancer seen in diabetics over that seen in the general population. The preliminary data discussed in the symposium suggested lower rates of cancer with Levemir (insulin detemir; Novo Nordisk) compared to neutral protamine Hagedorn (NPH), but higher rates with Lantus.

Since the EASD announcement, there has been little change in the use of Lantus due to the weakness of the four insurance studies published in June 2009, but the presentation of data at the EASD to such a large audience of specialists may lead to a change in prescription patterns. It is unlikely that existing patients will be switched due to the biovariability differences between Lantus and Levemir and the loss of glycemic control during titration, but physicians may think twice when initiating a patient on a long-acting insulin. Long-term implications of the cancer scare may involve a greater focus on reducing the insulin requirements of patients through reductions in beta-cell function decline and insulin sensitivity, rather than using insulin as a panacea to elevated glycated hemoglobin A1c (HbA1c) levels.

Dr Jay Skyler of the University of Miami, presenting a defense of Lantus on behalf of Sanofi-Aventis, highlighted the caution taken by the EASD when analyzing the data and making its recommendation that patients continue using Lantus. However, he was critical of the role of the lay-media in sensationalizing the story without reporting the caveats included within the original EASD press release. Dr Skyler also quoted from a rebuttal of the dose adjustments used within the studies which stated that the technique was flawed and the potential link between Lantus and cancer was unwarranted.

Analysis of data from a clinical trial to determine the impact of Lantus therapy on rates of diabetic retinopathy found no increase in malignancy and the medical ethics committee of the ORIGIN clinical trial found no reason to discontinue Lantus use in the trial due to increased malignancy rates. Dr Skyler concluded that Lantus was safe to use and that there was no link between the drug and cancer. Prior to the EASD meeting, however, Sanofi-Aventis announced that it will be undertaking a retrospective cohort study in Northern Europe and an epidemiological study in the US to determine the safety of Lantus.

Following the defense of Lantus, Dr David Russell-Jones from the University of Surrey, UK, offered a defense of the other long-acting insulin analog on the market, Levemir. Having not been included in the original insurance database studies, it was unknown whether Levemir would demonstrate similar characteristic to Lantus. A meta-analysis of the Novo Nordisk clinical trials database demonstrated a cancer event rate of 0.36 events/100 years exposure with Levemir compared to 0.92 events/100 years exposure with NPH. Data from a meta-analysis of Levemir and Lantus produced event rates of 0.87/100 years and 1.27/100 years exposure, respectively. Both of these results, however, were insignificant.

Dr Russell-Jones then focused on preclinical studies of the insulin analogs, long-acting as well as short-acting, which had been conducted by Novo Nordisk. These studies demonstrated that Lantus bound with significantly greater affinity to the IGF-1 receptor than Levemir, NovoRapid (insulin aspart; Novo Nordisk) and Humalog (insulin lispro; Eli Lilly), and that Levemir had lower mitogenic impact on selected cell lines than human insulin.

An initial study of 127,000 patients in an insurance database in Germany raised the possibility of a greater incidence of cancer in patients receiving Lantus compared to traditional human insulin NPH. This was submitted to Diabetologia, the journal of the EASD, but the editors treated the paper with understandable caution, wary of causing an unnecessary safety scare. Consequently, the editors delayed the publication of the paper and asked for confirmation of the study through similar studies of databases in Sweden, Scotland and the UK. The Swedish study found a statistical link between Lantus and breast cancer while the Scottish study identified a non-statistical link with breast cancer. In the UK study, meanwhile, no link was found. Importantly, the four studies only looked at the safety of Lantus compared to NPH with no other insulin analogs investigated.

A number of criticisms and weaknesses of the studies have been highlighted, primarily the make up of the patient populations. After adjusting for a number of variables, the patient populations in the Lantus and NPH groups were different and the pre-treatment characteristics of the groups may result in the differences seen in cancer risk. Additionally, the low number of cancer events in the Swedish and Scottish studies could mean that they were a result of chance.

During the symposium, Dr Ulf Smith, chairman of the EASD, and Dr Edwin Gale, editor of Diabetologia, outlined a potential mechanistic pathway for insulin to have mitogenic properties. Surgery

If joint condition has been compromised to the point that bone is scratching on bone, joint replacement surgery may be performed to improve the quality of life. This treatment is not common, but is an option for patient with both traditional arthritis and psoriatic arthritis. As both psoriasis and arthritis are incurable, the surgery is only a temporary treatment that will reduce pain for a given length of time. Aging patients may choose to opt for surgery if the joints have deformed to the point where they are immovable even if pain is not the major symptom.

Each of these medical treatments offers an additional step toward symptom management, but none of these treatments are prescribed as a standalone option. Doctors have quickly learned the power of alternative therapies and lifestyle changes on the effects of psoriatic arthritis on the body. Most often, medical treatment will be prescribed in conjunction with alternative therapy and changes.

Cervical spine is made up of seven separate bones called vertebrae. Each vertebrae has its own functions to perform and hence the shape of each vertebrae varies.

In the front ,a disc of soft elastic but strong tissue of 8m thickness is interposed between two vertebrae which is known as inter vertebral discs. THE FUNCTION OF THE SPINE:

The spine of the humans is called to transmit weight of the body to the ground. The body of the vertebrae do the task of transmission of weight.

In the cervical spine the convexity is towards anterior side .The maximum point of convexity is at the level of disc between fifth and sixth cervical vertebrae. The lamina of the consecutive vertebrae form the spinal canal through which the spinal cord passes downwards. The spinal canal protects the cord from the external injurious forces

Degenerative changes are common in the cervical spine. The degenerative changes affects the inter vertebral discs first and the posterior inter vertebral joints secondarily ,causing pain and stiffness of the neck, sometimes refered to the upper limb. It is common in the middle aged persons and in the people doing prolonged work with neck in flexed position

CAUSES:

The primary degenerative changes are often initiated by injury, repeated minor trauma or the condition is the manifestation of the widespread degenerative changes that occur with increasing age.

PATHOLOGY:

Degenerative arthritis commonly occurs in the lower three cervical joints. First the central inter vertebral joints are affected later , posterior inter vertebral joints are involved.

In the centaral joints there is degenerative narrowing of the inter vertebral discs and bone reaction at the joint margins leads to the formation of the osteophyte(spurs).These spurs exerts pressure on the cranial nerves and lead to the pressure symptoms

CLINICAL FEATURES:

1. Neck symptoms consist chiefly of aching pain in the back of the neck or in the trapezius area.

2. Occipital head ache may be feature if the upper half of the cervical spine is involved

3. Symptoms are severe if the cervical nerves are interfered.

4. The main feature of the nerve root irritation is the radiating pains along the course of the nerve often reaching the digits. There may be paraesthesia in the hand ,in the form of tingling or pins and needles.

5. Noticeable muscle weakness is common

6.Osteophytes arising from the anterior vertebral margins may sometimes give rise to dysphagia

RADIOGRAPHIC EXAMINATION:

There is narrowing of the space of the inter vertebral disc, with formation of the osteophytes at the vertebral margins.

COMPLICATIONS:

If the spinal canal is markedly narrowed by the osteophytes ,it may cause paralysis of the four limbs and bladder,but it is very uncommon
